We are not currently accepting any new patients with Medicaid products (Aetna Better Health, Anthem Healthkeepers Plus, United Healthcare Community Plan). We do not accept self-pay patients.
CIGNA CONNECT Patients: We are a participating provider and accept new patients with Cigna Connect, however if you need any services outside of our office (surgery, imaging, rehab, etc…) we cannot guarantee that the facility participates with Cigna Connect.
UNITED HEALTH CARE Patients: Please be aware of your insurance’s requirements for referrals, it is your responsibility to ensure that a valid referral is on file with our office at least 24 hours prior to your appointment.
