Audiology Center
We provide advanced evaluation of pediatric and adult hearing, including traditional audiometry, evoked potentials (OAE) and electrophysiologic evaluations (neurodiagnostic ABR, ECOG). We offer treatment of pediatric and adult hearing loss with the latest hearing aid technology that pairs clarity with discreet design. We also offer comprehensive treatment of tinnitus (ringing in the ears) through hearing aids and sound therapy.

What will I need for my first appointment?
Please complete our New Patient forms (below) and bring them to your appointment, or bring your medical, insurance and payment information and arrive early for your first appointment to complete your paperwork. Please bring any results from lab work, copies of x-rays or imaging, notes from other doctor appointments, and a record of your symptoms and treatment (for frequent or recurring issues).
Help us make your first appointment run smoothly. Please bring:
- Health Insurance Card/Information
- Drivers License or government-issued photo ID
- Paper Referral, if required by your insurance company
- Any Co-Pay required by your Insurance Plan
- A list of all medications you are taking (including the dosage and instructions)
- A copy of any X-Rays or CT scans (Please get the actual films burned onto a disc, not just the written report)
- Copies of records from doctors related to your sinuses/nose/allergies/lungs.
- Including, any recent hearing test results
- A list of your questions /concerns
- Complete the “New Patient” paperwork below

At this time, we are no longer accepting new Medicare, Medicaid or Tricare patients. Also, we do not accept self-pay patients.
CIGNA CONNECT Patients: We are a participating provider and accept new patients with Cigna Connect, however if you need any services outside of our office (surgery, imaging, rehab, etc…) we cannot guarantee that the facility participates with Cigna Connect.
UNITED HEALTH CARE Patients: Please be aware of your insurance’s requirements for referrals, it is your responsibility to ensure that a valid referral is on file with our office at least 24 hours prior to your appointment.
Do I need to see an ENT?
ENTs are otolaryngologists, doctors with specialized training to treat the ear, nose and throat. These areas might not seem like a large part of your body, but health concerns with these areas cause more than half of the health concerns that send people to the doctor’s office in the first place. Seeing an ENT often gets you to a diagnosis and treatment faster.
If you are experiencing chronic problems with your ears, nose or throat it is time to schedule a visit. If your allergies, runny nose, sinus headaches, sleep or balance issues are not responding to treatment by your primary care physician then it is time to schedule a visit. If you are having problems staying awake during the day and snoring wakes others at night then it is time to schedule a visit.
What is sinusitis and how is it treated?
Sinusitis is an inflammation of the sinuses, which may be due to infection. Sinusitis can be acute or chronic. Patients usually feel a combination of pain, pressure or headache. Sinus surgery is often necessary when sinusitis does not get better with medication or other treatments.
Will I need surgery?
Our office provides many types of treatment, including surgery, which is often a final recommendation when other options are not effective. In addition to being skilled diagnosticians and clinicians, ENTs are required to undergo surgical training. So, if you need a surgical procedure, the ENT you see in the office will be the surgeon you have in the operating room. Most ENT surgery is outpatient or day surgery, meaning you return home the same day as your surgery.
